According to a recent announcement by Qatar’s Ministry of Public Health (MoPH), visitors to Qatar are required to have a health insurance policy starting from 1 February 2023.

Qatar’s Law No. (22) of 2021 regulating healthcare services in Qatar stipulates that visitors must be covered by a mandatory health insurance scheme. The mandatory health insurance scheme will be implemented in phases, and the first phase has started on 1 February 2023 .

Starting 1 February 2023, all visitors to Qatar applying for a visitor’s visa are required to have health insurance policies. This is mandatory for all types of visit visas including Family Visit Visa , GCC Residents Visit Visa , and Visa On Arrival .

AUGUST 2023 UPDATE: According to Qatar Government’s Hukoomi helpline, visitors traveling on an instant visa and staying in Qatar for less than 30 days do not need to obtain Health insurance . However it is highly recommended to have insurance as it can be helpful in case of an emergency.

GCC citizens are exempted from the mandatory health insurance while entering Qatar.

According to MOPH, the health and well-being of visitors are of paramount importance. Therefore the Government has established the necessary regulations to ensure that visitors are protected against accidents and medical emergency-related conditions during their stay.

By implementing a sustainable and effective healthcare system, and by regulating healthcare expenditures, the scheme aims to regulate health insurance and ensure continuous improvements.

Visitors Applying For Visit Visa Online

  • Visitors applying for a visit visa online via the Metrash App will be directed to the MOPH website.
  • There, they can select one of the insurance companies registered with the Ministry to purchase the visitors’ policy after completion of all other relevant visa requirements.
  • Once the insurance policy is issued by the selected insurance company, the Ministry of Interior will issue the appropriate visit visa.

The Visitor will receive the Health Insurance Policy from the website of the Insurance companies. The policy can be printed, kept and shown when needed.

Visitors Eligible For Visa On Arrival

To ensure speedy completion of your procedures upon arrival at the various border crossings (airport, land and seaports), visitors are advised to purchase a visitor insurance policy before arriving at these ports from the MOPH link provided below and follow the same procedures mentioned.

The registered insurance companies are listed on the Ministry of Public Health’s website. Visitors can choose any one of the listed insurance companies through the links available on MOPH website:

Health Insurance For Visitors To Qatar Authorised Companies

MoPH has indicated that for visitors who hold international health insurance, the insurance policy must include Qatar, be valid during their stay in the country and be issued by one of the insurance companies approved in Qatar.

Cost of Visitor’s Health Insurance Policy:

The standard premium for a single insurance policy will be QAR 50 for 30 days (USD 13.73). For the extension of a visit visa beyond 30 days, the visitor will have to buy an additional policy.

In addition, visitors can obtain additional health insurance policies, and the premiums for these policies will vary depending on the insurance company’s prices.

Health insurance premiums cannot be refunded after the purchase of the policy if the stay duration is less than the period of the visa.

The effective date of coverage is the date of entry of the visitor at any border.

If the visitor wishes to extend his/her stay in the State of Qatar, then he/she must purchase a new health insurance policy. The minimum duration of the visitor’s insurance policy is 30 days, it is not possible to purchase a policy with less than this duration.

The basic visitor insurance policy in case of a single-entry visa expires upon leaving the State of Qatar through any air, sea or land ports.

There is an insurance policy for multiple-entry visas and it remains valid from the date of entry until the policy expires.

Coverage Under Visitor’s Health Insurance Policy

The mandatory health insurance for visitors will only cover emergency medical treatments and accidents .

​Emergency Medical Treatment:

  • Up to QAR 150,000 for the policy period and within the State of Qatar.
  • Emergency medical assistance with a sub-limit of QAR 35,000, includes:
  • Emergency ambulance transportation within the State of Qatar, and where necessary.

COVID-19 and Quarantine:

  • Sub-limit up ​to QAR 50,000.
  • Covid-19 treatment for positive cases.
  • Quarantine expenses (for confirmed cases) are up to QAR 300 per day.

Repatriation:

In the event of the visitor’s death within the State of Qatar, the cost of repatriation is covered up to an amount of QAR 10,000.

Is there a waiting period for this policy to be active?

No, there will be no waiting period for the visitor’s health insurance policy.

Does the policy require any copayment or deductible?

No copayment or deductible will be required for any covered services as per the terms and conditions stipulated in the visitor’s health insurance policy.

In addition to the listed insurance companies, Qatar MOI may accept some international health insurance companies, subject to the fulfilment of the following conditions and requirements set by the Ministry of Public Health to be accepted for entry visa issuance:

  • To be issued by one of the insurance companies included in the approved list by the Ministry of Public Health.
  • Its geographical coverage includes the State of Qatar.
  • To be valid and cover the period of stay in the country.
  • To cover emergency services and accidents with an annual limit of 150,000 Riyals or more, without deductibles or excesses.
  • To include a QR code.

The policy can be uploaded to the website of the Ministry of Interior or a paper copy to be submitted to the competent officer at the border crossing.

Dear ALL, I am writing to inform you that I have recently obtained approval from METRASH for my mother’s application. I have also received a confirmation SMS regarding this matter. Hi OB, For health insurance, you are not paying through Metrash. It should be paid directly to the insurance company and it will be updated in the system (through visa number). When you receive the visa approval SMS there will be a link to the MOPH website from where you can select an insurance company and buy insurance directly. This will take some time (less than 24 hours) to update in the MOI system. The visa payment and visa print option will work after this only. Hi Asma, As per the updated rules, health insurance is not needed for visits less than 30 days. However some airline staff don’t know this rule and will ask for insurance. If you are being insisted you can purchase insurance using your mobile for QR 50 (within 5 minutes). Note: If you are taking Family Visit Visa insurance is compulsory (your sponsor will get an SMS to take insurance).

Hi Smijesh, Even though the Hayya portal says insurance is required till January, most airlines are accepting travellers with insurance until return flight date. We never know when they will become strict about this. If they insist on insurance for full duration, you can take additional insurance online while at the airport.

How To Get Travel Health Insurance For Visitors To Qatar?

Are you planning a trip to Qatar? Starting from February 1, 2023, visitors to Qatar are required to have Travel Health Insurance. Law No. 22 of 2021 on Health Insurance

  • Qatar’s Law No. (22) of 2021 regulating healthcare services in Gytar stipulates the requirement of Health insurance to travel to Qatar. 
  • The law requires Qatari companies to give health insurance coverage to their foreign national workers who live and work in the country through private insurance companies. (Art. 11.) 
  • Foreign nationals’ work permits cannot be extended or renewed without proof of health insurance coverage. (Art. 10.)
  • In contrast to foreign nationals, Qatari citizens are not forced to use services provided by commercial health insurance firms under Law No. 22 but are entitled to free healthcare services from the Qatari government. (Art. 6.)

Mandatory Health Insurance for Qatar Requirement

All travellers to Qatar who applied for a visitor’s visa after 1 February 2023 are obliged to purchase health insurance. This is required for all types of visit visas, including the Family Visit Visa, the GCC Residents Visit Visa , and the Visa On Arrival. While GCC citizens are spared from the health insurance requirement during their visit to Qatar.

The Ministry of Public Health stated that tourists who have foreign health insurance must also include Qatar, must be valid throughout their stay in the country, and must be supplied by one of the insurance firms permitted in Qatar.

Prices of Visitor’s Health Insurance Policy: 

  • The regular price for a single health insurance policy for 30 days (about 4 and a half weeks) will be QAR 50.
  • A traveller must purchase extra health insurance to extend a visit visa beyond 30 days.
  • Travellers can also purchase extra health insurance plans, with varying rates based on the insurance company’s costs.
  • If the duration of the stay is shorter than the duration of the visa, health insurance premiums cannot be repaid.

Exemptions and Recommendations

Visitors to Qatar may be exempt from the mandatory health insurance requirement if their stay is less than 30 days. However, it is highly recommended to purchase health insurance even for short trips as some airline staff may ask for it. If insisted, insurance can be purchased for QR 50. Australian citizens, who can get a visa on arrival, usually do not need health insurance for short-duration trips. Health Insurance Validity Period

  • Health Insurance Validity Period for Visitors to Qatar
  • The visitor’s entrance date at any border is the date of commencement of insurance.
  • If the tourist chooses to prolong his or her stay in the State of Qatar, a new health insurance coverage must be purchased.
  • The tourist’s insurance coverage has a minimum period of 30 days (about 4 and a half weeks); a policy with a shorter duration cannot be purchased.
  • In the situation of a single-entry visa , the basic visitor insurance package expires upon exiting the State of Qatar whether it’s air, sea, or land port.
  • For multiple-entry visas , there is insurance coverage that is effective from the date of entrance until the policy expires

Hayya Cardholders and their Companions:  

Hayya cardholders and their companions must get visitors’ insurance plans that cover the remaining duration of the Hayya card entrance permit, which expires on January 24, 2024. For example, if a Hayya Cardholder intends to join the State of Qatar on March 1, 2023, he or she should buy an insurance plan for 11 months, and if intends to enter Qatar on April 1, 2023, he or she needs to purchase an insurance policy for 10 months, and so on.

Visitors from Abu Samra:  

In addition to the mandatory Health Insurance, visitors from Abu Samra must get automobile insurance. Vehicle insurance may be purchased online.

Coverage under the Visitor’s Health Insurance Policy

Visitors’ compulsory health insurance is limited to covering emergency medical procedures and accidents.

Emergency Medical Assistance:

• Up to QAR 150,000 throughout the policy and inside the State of Qatar.

• Emergency medical care with a sub-limit of QAR 35,000 includes Emergency ambulance transportation inside Qatar and anywhere necessary.

COVID-19 and Quarantine: 

• Sub-limit up to QAR 50,000.

• Treatment of Covid-19 in positive cases.

• The cost of quarantine (for confirmed illnesses) could exceed QAR 300 per day.

Repatriation: 

If a tourist dies within the State of Qatar, the expense of repatriation will be covered up to a maximum of QAR 10,000.

Is there a waiting time before this policy is activated?

No, the visitor’s health insurance coverage will not have a waiting period.

Is there a copayment or deductible related to this policy?

According to the terms and conditions of the tourist’s health insurance policy, no copayment or deductible will be paid for any covered services.

In the event of an emergency, what should I do?

You can go to a service provider who is part of the insurance company’s network. The policyholder can contact the registered insurance company for assistance in locating the nearest treatment centre that can offer the necessary care.

Acceptance Criteria for International Health Insurance Companies:

Besides the above insurance providers, Qatar MOl may accept some foreign health insurance companies if the following rules and regulations established by the Ministry of Public Health are met:

1. Must be granted by one of the insurance providers on the Ministry of Public Health’s authorized list.

2. The State of Qatar is included in its geographical coverage.

3. It must be valid and cover the duration of the visitor’s stay in the country.

4. To pay for emergency services and accidents up to 150,000 Qatari Riyals per year, with no deductibles or surcharges.

5. Must Include a QR code. 

The policy can be posted to the Ministry of Interior’s website or printed and shown to the designated officer at the point of entry.

The Qatar tourist visa is available to nationals of all countries, except GCC citizens, but citizens of over 101 countries are eligible for a free visa on arrival (see Visa-free entry above). Travellers of select nationalities will require to pay a fee to obtain a visa on arrival or apply for a tourist visa prior to travel. Visas can be applied for on the  Hayya platform for a cost of QAR 100.

Applying for a visa using the Hayya platform

Thanks to a host of visa facilitation measures, Qatar is now the most open country in the Middle East. Visitors from all around the world can now enter Qatar either by obtaining visa on arrival, or by filling out simple online applications, depending on the passport they carry.

Hayya card validity extended until 2024

Are you a Hayya Card holder living outside the State of Qatar? We’ve got news! 

The validity of the Hayya Card has been extended and will now allow Hayya Card holders to enter the State of Qatar until February, 2024.

Travel requirements: 

- Proof of a confirmed hotel reservation or accommodation with family or friends approved through the Hayya portal

- Passport valid for no less than three months upon arrival in the State of Qatar 

- Valid health insurance during the period of stay in the country. We recommend all travellers visiting the State of Qatar to have a valid health insurance policy covering Qatar's medical facilities. Those who don’t already have an insurance policy are welcome to purchase one on arrival in the State of Qatar. For more information on travel insurance, click  here .

- Return tickets

All Hayya card holders visiting Qatar will have access to the following:

- The 'Hayya with Me' feature' allowing Hayya Card holders to invite up to three family members or friends

- A multiple-entry permit 

- No fees will apply

Qatar Transit Visa

Qatar Airways passengers transiting in Hamad International Airport for a minimum of 5 hours are eligible to apply for the Qatar Transit Visa. It is free of charge, valid for up to 96 hours (four days) and open to passengers of all nationalities**.

Nationals of the Gulf Cooperation Council countries (Bahrain, Kuwait, Oman, Saudi Arabia and United Arab Emirates) do not require a visa to enter Qatar.

Mandatory health insurance

We recommend all travellers visiting the State of Qatar to have a valid health insurance policy covering Qatar's medical facilities. Qatar Travel Insurance Requirements

As of February 1, 2023, all foreign visitors are required to have an approved and recognized medical insurance policy in order to gain entry into Qatar. The policy must be effective for the duration of their stay, and can be purchased in advance or on arrival at a cost of 50 Qatari Riyal, roughly $14.

According to authorities, travelers with international health insurance must have a policy that’s valid in Qatar, covers their entire stay in the country, and was issued by one of the recognized insurance providers. Be sure to visit the official Visit Qatar website for a full list of recognized insurance providers.

Squaremouth’s Travel Insurance Recommendations

All foreigners visiting Qatar must purchase a policy from a insurance provider that’s been recognized by the Qatari government in order to gain entry into the country. With that said, there’s a possibility that the coverage options available from those recognized providers won’t meet the needs of some travelers. If additional coverage is needed, Squaremouth recommends supplementing the required coverage with a comprehensive travel insurance plan.

For travel to Qatar, Squaremouth recommends a policy with at least $50,000 in Emergency Medical coverage and $100,000 in Medical Evacuation coverage. This is in line with Squaremouth’s general international medical recommendations, due to the potential for high medical expenses overseas.

Emergency Medical coverage can offer reimbursement for expenses due to an unforeseen illness or injury. Covid-19 treatment can also be covered under most policies. The Medical Evacuation benefit can provide reimbursement for costs associated with transportation for a traveler to get to the nearest adequate medical facility.

Travelers visiting Qatar may have extensive travel arrangements, pre-paid trips costs, and non-refundable expenses, such as airfare, accommodations, tours, sports tickets, or excursions. For these travelers, Squaremouth recommends a comprehensive plan that offers the Trip Cancellation benefit. This benefit can reimburse up to 100% of these prepaid and non-refundable insured trip costs if a traveler has to cancel for a covered reason.

The most common covered reasons typically include contracting Covid-19 or another unforeseen illness, an injury, inclement weather, or terrorism.

Policies with Trip Cancellation are comprehensive. This means these policies often include coverage for other major traveler concerns, such as delayed baggage, missed connections, or a trip that gets interrupted after the departure.

Health insurance is mandatory for visitors and arrivals to Qatar as one of the conditions for issuing or renewing entry visas for expatriates or visitors, granting or renewing a residence permit, and arrivals may only be employed after providing proof of their participation in compulsory health insurance for the duration of their stay, following the issuance of Law No. 22 of 2021 regulating health care services.

The Ministry of Public Health confirmed that under the new law, a mandatory health insurance system will be implemented for all those who come to the country and visitors to the country, which provides basic health care services to them through health care providers in a number of government health facilities and the private sector, in accordance with their insurance coverage system.

The provisions of the Act also require the employer to provide health insurance to non-Qatari workers through insurance companies registered with the Ministry of Public Health, in order to provide them with basic insurance coverage, and the same obligation applies to the applicant to provide basic coverage to the recruited. So what about prices?

From QR 495 to QR 920

A source confirmed to “Al-Sharq” in one of the insurance companies that the annual insurance price for individuals under 60 years of age ranges from QR 495 to QR 920, while the annual insurance coverage rate ranges from 60% to 80% based on the insurance package determined by individuals, companies or groups.

For individuals over 60 years of age, they range from QR 495 to QR 900, with annual insurance coverage ranging from 60% to 70%, and health insurance packages include basic services and emergency services in the under-60 category, and chronic disease coverage, but it depends on the coverage rate based on the selected insurance package, with the identification of health facilities that accept insurance.

Some insurance companies have also allocated packages within the reach of individuals amounting to QR 499, as well as facilities for families in order to facilitate access to visitors and arrivals for basic health services, while one of the renowned insurance companies has approved the allocation of QR 153 for visitors for three months to cover the visitor for emergencies up to QR 180,000 and covid-19.

يعتبر التأمين الصحي إلزاميا للزوار والوافدين إلى دولة قطر أحد شروط إصدار سمة دخول لوافد أو زائر، أو تجديدها أو منح ترخيص الإقامة أو تجديدها، كما لا يجوز توظيف الوافدين إلا بعد تقديم ما يثبت اشتراكهم في التأمين الصحي الإلزامي طوال مدة الإقامة، وذلك بعد إصدار القانون رقم (22) لسنة 2021 المنظم لخدمات الرعاية الصحية.

وزارة الصحة أكدت أنه بناءً على القانون الجديد سيتم تطبيق نظام تأمين صحي إلزامي لجميع الوافدين للدولة والزائرين لها، والذي يقدم خدمات الرعاية الصحية الأساسية لهم عبر مقدمي خدمات الرعاية الصحية في عدد من المرافق الصحية الحكومية والقطاع الخاص، وفقاً لنظام التغطية التأمينية لهم.

كما أن أحكام القانون تلزم صاحب العمل بعمل تأمين صحي للعاملين غير القطريين لديه عن طريق شركات التأمين المسجلة لدى وزارة الصحة العامة، وذلك لتوفير التغطية التأمينية الأساسية لهم، وينطبق نفس الالتزام على المستقدم لتوفير التغطية الأساسية لمن يستقدمهم.. فماذا عن الأسعار؟

من 495 ريالاً إلى 920 ريالاً

أكد مصدر لـ”الشرق” في إحدى شركات التأمين أنَّ سعر التأمين السنوي للأفراد تحت الستين عاما يتراوح ما بين 495 ريالا قطريا إلى 920 ريالا قطريا، أما نسبة التغطية التأمينية السنوية فتتراوح ما بين 60 % – 80 % بناء على الباقة التأمينية التي يتم تحديدها من قبل الأفرد أو الشركات أو المجموعات.

وبالنسبة للأفراد فوق الستين عاما فتتراوح ما بين 495 ريالا قطريا إلى 900 ريال قطري، بنسبة تغطية تأمينية سنوية تتراوح ما بين 60 % – 70 %، وتتضمن باقات التأمين الصحي تغطية الخدمات الأساسية والحالات الطارئة في الفئة تحت الـ60 عاما، وتغطية الأمراض المزمنة إلا أنها تعتمد نسبة التغطية بناء على الباقة التأمينية المُختارة، مع تحديد المرافق الصحية التي تقبل بالتأمينز

كما خصصت بعض شركات التأمين باقات في متناول الأفراد بلغت 499 ريالا قطريا إلى جانب تسهيلات للعائلات بهدف تيسير وصول الزوار والوافدين للخدمات الصحية الأساسية، فيما أقرَّت إحدى شركات التأمين الذائعة الصيت تخصيص مبلغ 153 ريالا قطريا للزوار لمدة 3 أشهر لتغطية الزائر على الحالات الطارئة لحد 180 ألف ريال قطري والإصابة بفيروس كورونا “كوفيد- 19”.

Entry requirements

This advice reflects the UK government’s understanding of current rules for people travelling on a full ‘British citizen’ passport from the UK, for the most common types of travel.

The authorities in Qatar set and enforce entry rules. If you’re not sure how these requirements apply to you, contact Qatar’s embassy in the UK .

COVID-19 rules

Countries may restrict travel or bring in rules at short notice. Check with your travel company or airline for changes.

If you test positive for COVID-19, you may need to stay where you are until you test negative. You may also need to seek treatment there.

Travel to Qatar

There is information about Qatar’s COVID-19 travel and return policy on Qatar’s Ministry of Public Health website .

You may need to have thermal screening or temperature checks when you arrive in Qatar at air, land or sea ports.

If you are intending to stay in Qatar for 30 or more days, you must get health insurance before travelling. You must buy an insurance policy from a  registered Ministry of Public Health insurance provider (MOPH).

All visitors are advised to get travel insurance before entering Qatar. For more guidance see the MOPH website.

The standard insurance premium is 50 Qatari riyals per person, per month.

Public spaces and travel within Qatar

Qatar’s Ministry of Public Health has published advice on how to limit the spread of coronavirus .

You must wear face masks on public transport, and to enter healthcare facilities.

To enter any public and private healthcare facility, you must present your COVID-19 healthcare status via the Ehteraz app, available from the apple app store or the google playstore .

If you break the rules, punishments could include a fine of up to 200,000 Qatari riyal or a prison sentence of up to 3 years.

Passport validity requirements

If you are visiting Qatar, your passport should be valid for at least 6 months from the date you arrive.

If you are a resident of Qatar, your passport must be valid when you arrive.

Visa requirements

You can get a tourist visa on arrival if you’re travelling on a full British Citizen passport.

You must still get a visa if:

  • you’re travelling for any purpose other than tourism
  • you hold another type of British passport

If you need to stay longer than 30 days, you must extend your visa waiver before it expires through the Qatar Ministry of Interior . If you do not do this, you may be fined. The fine must be paid before leaving Qatar.

Vaccination requirements (other than COVID-19)

At least 8 weeks before your trip, check the vaccinations and vaccination certificates you may need on TravelHealthPro .

Customs rules

There are strict rules about what goods you can take in and out of Qatar. You must declare anything that may be prohibited or subject to tax or duty.

Importing drugs, alcohol, pornography, pork products and religious books and material into Qatar is illegal. All luggage is scanned as you enter the country. DVDs and videos may be examined, censored and confiscated.

Qatar law also prohibits the importation, sale and purchase of electronic cigarettes, liquids and other similar products (e.g. electronic shisha pipes). The law applies regardless of quantity and intended use. Customs officials may seize and confiscate any such items found entering the country by any means, including in passengers’ luggage or sent by post.

If you are taking money, precious metals and stones into or out of Qatar which are worth more than 50,000 Qatari riyals, you must complete a declaration form. This includes:

  • currency – Qatari riyal or foreign currencies
  • financial bearer negotiable instruments – such as travellers cheques, money orders, cheques, promissory notes (including those endorsed without restriction, incomplete, payable to a fictitious payee or signed but without the payee’s name)
  • precious metals – such as gold, silver, platinum
  • precious stones – such as diamonds, emeralds, rubies, sapphires, pearls

You must also give customs officials any other information they request.

If you break these rules, you could be fined or imprisoned, as well as the funds being seized.

Transiting through Qatar

At the airport, all bags are scanned. Transiting passengers carrying even residual amounts of drugs may be arrested. This includes medication which may be legal in other countries, but not in Qatar. See bringing medication into Qatar

Travellers cannot stay in the transit area for more than 24 hours between flights, even if you are using the transit hotel in the airport.

Will Medicare raise your monthly premiums to cover Wegovy?

Medicare’s recent move to cover the weight-loss drug Wegovy for some recipients with heart disease risk could drive up the cost of monthly premiums for many of the 65 million adults enrolled in the federal health insurance program, experts warn. 

How much of a price jump isn’t yet known. The premium increase would depend on how many millions qualify for the popular medication, which is priced at around $1,300 a month . 

It’s possible premiums could go up as soon as 2025, although 2026 or beyond is more likely, according to at least one health policy expert.

“I worry greatly, both in terms of higher premiums and also Medicare funds over the next decade,” said Lawrence Gostin, director of the O’Neill Institute for National and Global Health Law at Georgetown University. “The drug needs to be taken lifelong to be effective, so we’re looking at potentially enormous lifelong costs.”

The drug will be covered under Medicare Part D, which pays for prescription drugs people take at home, according to the Centers for Medicare and Medicaid Services. Last year, more than 50 million of Medicare’s enrollees used Part D plans, according to KFF , a nonpartisan group that studies health policy issues. 

This year, people on Medicare Part D pay an average monthly premium of $55.50, according to the CMS . After they meet a so-called coverage gap, patients are on the hook for 25% copays until they spend a certain amount out of pocket, which varies by plan. Part D plans are provided by private insurance companies that have contracts with the U.S. government.

A handful of Part D providers, including CVS Health and Kaiser Permanente, have already said they would start covering Wegovy for patients with heart disease risk. Every major insurance company has been asked for comment.

Under the CMS’ guidelines, Wegovy can be prescribed only to people with heart disease who are either overweight or obese. Medicare won’t pay for Wegovy if it’s being used only for weight loss. 

It’s not yet clear how many on Medicare meet those criteria. Juliette Cubanski, deputy director of the program on Medicare policy at KFF, estimated that it is most likely “several million people.”

“If costs go up, then the contribution from the federal government goes up, and generally speaking, beneficiary premiums also go up,” she said.

It’s unlikely that a rise in monthly premiums linked to Wegovy will happen within the next year, said Jeffrey Davis, the health policy director at McDermott and Consulting, a group that provides data analytics and policy advice to the health care industry. 

That’s because, he said, private insurance companies that provide Part D coverage have already begun locking in payment rates with Medicare for the 2025 enrollment year.

On the other hand, insurers could still jack up premiums next year if early projections of Wegovy’s costs come in higher than expected, he said. 

An analysis published by KFF last month found that gross Medicare spending on the diabetes drug Ozempic, which shares the same active ingredient as Wegovy, increased from $2.6 billion in 2021 to $4.6 billion in 2022, making it the sixth top-selling drug in Medicare Part D that year. Medicare began covering Ozempic in 2018. 

However, Davis added, any kind of premium increase caused by Wegovy is much more likely to happen as early as the 2026 enrollment year. 

“This drug is expensive,” Davis said. “If millions of people are covered, it could increase premiums going forward.”

Cubanski said even a few dollars in higher premiums can be a big deal for people on Medicare who often are retired and have limited incomes.

Barriers to coverage

It’s important to note that Medicare is allowing insurers that provide Part D coverage to establish rules before they cover Wegovy, a practice that might restrict access in the near term and could keep premiums low, said Gretchen Jacobson, vice president of the Medicare program at the Commonwealth Fund. 

Those rules include step therapy, which requires patients to try lower-cost medications before they are allowed to try medications that cost more, Gretchen said.

Some commercial insurers, for example, might start patients off with an older weight-loss medication before they’re allowed to get prescribed a GLP-1 drug.

Insurers can also require Medicare Part D patients to get prior authorization from their insurance companies before they can get the medication.

Dr. Amit Khera, a fellow at the American Heart Association, said that prior authorization is typical for other expensive heart medications and that it’s often a burdensome process for both providers and patients who have to regularly submit medical records and tests showing they meet the requirements for drugs.

“I think the intent is sort of to ensure the right patient gets the right medicine, but practically, it ends up just being an incredibly cumbersome process that, in some ways, can restrict access,” Khera said.

Limiting costs

While insurance companies get a bad reputation for limiting access to patients, Novo Nordisk may also need to share blame for the high cost, Gostin said. 

He isn’t alone in that view: Sen. Bernie Sanders, I-Vt., the chair of the Health, Education, Labor and Pensions Committee, last month called on the drugmaker to lower the cost of the drug.

He cited a study by Yale researchers that found that Ozempic and Wegovy could cost less than $5 a month to make, even though Novo Nordisk charges $1,000 a month in the U.S. for semaglutide, the active ingredient in Wegovy. 

“I think Bernie Sanders is right to call on them to do that, particularly when it’s selling to Medicare enrollees,” Gostin said. 

Even if Novo Nordisk doesn’t lower the prices, semaglutide could be up for Medicare drug pricing negotiations under the Inflation Reduction Act as early as next year, Cubanski said. 

The negotiated prices wouldn’t go into effect until 2027.

Davis also said it’s possible insurers providing Part D coverage could negotiate better rebates — discounts drug companies give insurers in exchange for coverage — that could lower the costs for a period of time.

The Captive was created by statute in 2008 and administered by the Chief Risk Officer, Office of Risk Management (ORM). ORM incorporated the Captive and began writing medical malpractice insurance policies in FY 2008. The liability of the agency is limited to the funds available to the Captive's participants. In FY 2014, the scope of the Captive was expanded to include property insurance.
